Aerobic capacity and health-related quality of life in adults HIV-infected patients with and without lipodystrophy BJID
Gomes Neto,Mansueto; Conceição,Cristiano Sena; Ogalha,Cecília; Brites,Carlos.
Abstract Introduction HIV infection and its therapy which can affect their aerobic capacity and health-related quality of life of patients. Objective We conducted a cross-sectional study to determine if aerobic capacity and health related quality of life was decreased in HIV-infected patients receiving highly active antiretroviral therapy and comparing patients with and without lipodystrophy. Research design and methods HIV-infected patients older than 18 years, and in current use of highly active antiretroviral therapy drugs, were evaluated for blood count, fasting total cholesterol, high density lipoprotein, triglycerides, glucose, HIV viral load and CD4/CD8 counts, body composition, peak oxygen consumption (peak VO2) and metabolic equivalent. Health...

Aeromonas caviae septicemia in immunocompetent gastrointestinal carriers BJID
Dwivedi,M.; Mishra,A.; Prasad,A.; Azim,A.; Singh,R.K.; Baronia,A.K.; Prasad,K.N.; Dwivedi,U.N..
Aeromonas caviae strains have been isolated from blood and stool cultures of three immunocompetent patients, residents of Northern India, who presented with community acquired septicemia without any recent history of diarrhea. Cell culture infectivity test performed on Hep-2 cells have shown substantial degree of invasiveness in the isolated strains. This case unleashes a possibility of asymptomatic gastrointestinal carriage of such strains of A. caviae in a very large population of India, as several areas of India have very high rates of Aeromonas induced acute diarrhea/gastroenteritis (up to 13%). It needs to be appraised further in India as well as other countries having high rates of Aeromonas induced acute diarrhea/gastroenteritis.

Aeromycological study at the intensive care unit of the "Dr. Manuel Gea Gonzalez" General Hospital BJID
Ríos-Yuil,José Manuel; Arenas,Roberto; Fernández,Ramón; Calderón-Ezquerro,María; Rodriguez-Badillo,Raymundo.
INTRODUCTION: An aeromycological study verifies the presence and quantifies the concentration of fungal propagules in the air. It is very important in the hospital setting because of the increasing numbers of immunosuppressed and severely ill patients. The objective of this study was to determine the concentration of fungi in the air of the intensive care unit (ICU) of "Dr. Manuel Gea González" General Hospital. METHODS: This is a descriptive, observational cross-sectional study. Air samples were obtained with a single stage Thermo-Andersen Viable Particle Sampler (Thermo Electron Corporation -Massachusetts, U.S.A.) in a Petri dish with potato dextrose agar for 15 minutes at two different times (morning and afternoon) and heights (1 and 1.5 meters). The...

Aging with HIV: a practical review BJID
Cardoso,Sandra Wagner; Torres,Thiago Silva; Santini-Oliveira,Marilia; Marins,Luana Monteiro Spindola; Veloso,Valdiléa Gonçalves; Grinsztejn,Beatriz.
The worldwide elderly population is expected to grow by an additional 694 million people by 2025. By that time, there will be approximately two billion elderly people in the world, most of whom (80%) will be living in developing countries. Based on recent estimates, this population will number over 40 million in 2030 in Brazil and a consequent increase in governmental spending for this population can be expected. Since highly active antiretroviral therapy became available in the mid-1990s, the life expectancy of people living with HIV has increased significantly. Approximately 12 million life years were added to the world between 1996 and 2008 as a consequence of wider access to highly active antiretroviral therapy. In Brazil, the incidence of AIDS among...

Aging with HIV: an overview of an urban cohort in Rio de Janeiro (Brazil) across decades of life BJID
Torres,Thiago Silva; Cardoso,Sandra Wagner; Velasque,Luciane de Souza; Marins,Luana Monteiro Spindola; Oliveira,Marília Santini de; Veloso,Valdilea Gonçalves; Grinsztejn,Beatriz.
The introduction of highly active antiretroviral therapy during the 1990s was crucial to the decline in the rates of morbidity and death related to the acquired immunodeficiency syndrome (AIDS) and turned human immunodeficiency virus (HIV) infection into a chronic condition. Consequently, the HIV/AIDS population is becoming older. The aim of this study was to describe the immunological, clinical and comorbidity profile of an urban cohort of patients with HIV/AIDS followed up at Instituto de Pesquisa Clinica Evandro Chagas, Oswaldo Cruz Foundation in Rio de Janeiro, Brazil. Retrospective data from 2307 patients during January 1st, 2008 and December 31st, 2008 were collected. For continuous variables, Cuzick's non-parametric test was used. For categorical...

AIDS-associated diarrhea and wasting in northeast Brazil is associated with subtherapeutic plasma levels of antiretroviral medications and with both bovine and human subtypes of Cryptosporidium parvum BJID
Brantley,Richard K.; Williams,K. Robert; Silva,Terezinha M.J.; Sistrom,Maria; Thielman,Nathan M.; Ward,Honorine; Lima,Aldo A. M.; Guerrant,Richard L..
Advanced HIV infection is frequently complicated by diarrhea, disruption of bowel structure and function, and malnutrition. Resulting malabsorption of or pharmacokinetic changes in antiretroviral agents might lead to subtherapeutic drug dosing and treatment failure in individual patients, and could require dose adjustment and/or dietary supplements during periods of diarrheal illness. We determined the plasma levels of antiretroviral medications in patients that had already been started on medication by their physicians in an urban infectious diseases hospital in northeast Brazil. We also obtained blood samples from patients hospitalized for diarrhea or AIDS-associated wasting, and we found reduced stavudine and didanosine levels in comparison with...

AIDS-related lymphomas in Nigeria BJID
Bolarinwa,R.A.; Ndakotsu,M.A.; Oyekunle,A.A.; Salawu,L.; Akinola,N.O.; Durosinmi,M.A..
Aggressive non-Hodgkin's lymphoma (NHL), including primary central nervous system (CNS) lymphoma, lymphoblastic lymphoma and non-endemic Burkitt's lymphoma have been recognized as AIDS-defining cancers in most developed countries. However, HIV/AIDS epidemics appear not to have been associated with higher incidence of lymphomas in Africa. We therefore carried out this study to highlight the significance or otherwise of HIV/AIDS epidemics in the pathogenesis of lymphomas in a population of Nigerians with the disease. Since January 1993 to the present, all patients with haematologic cancers are routinely screened (following appropriate counseling) for HIV infection. Patients with a histological diagnosis of malignant chronic lymphoproliferative diseases...

Alteration in the erythrocyte sedimentation rate in dengue patients: analysis of 1,398 cases BJID
Souza,Luiz José de; Reis,Aldo Franklin Ferreira; Almeida,Fernanda Carlos Rodrigues de; Souza,Lara Assed de; Abukater,Marina; Gomes,Maurício Assed Estefan; Abicair,Olívia Araujo; Gonçalves,Pedro Assed.
A study of the erythrocyte sedimentation rate during the first hour (ESR) in dengue patients would help determine how this parameter is affected by this disease, as well as whether it can be used for diagnosis. One thousand, three hundred and ninety-eight cases of dengue attended at the Dengue Treatment Center, Campos dos Goytacazes Rio de Janeiro, Brazil, were included. The ESR values were classified as normal or elevated and compared by gender and clinical form of the disease. Among the 1,398 cases ESR was normal in 81.25% (n = 1,136), while in 18.75% (n=262) it was elevated. In 83.92% (n=514) of the male dengue patients ESR was normal, while in 16.08% (n=98) it was elevated. Among female dengue patients, 79.17% (n=622) had normal ESR, and in 20.83%...

INTRODUCTION: Type 3 dengue virus caused an extensive epidemic in the state of Rio de Janeiro in summer 2002. In some of the patients, it was found in an atypical form with increased aminotransferase levels and acute hepatitis. MATERIAL AND METHODS: An analysis was made of 1,585 serologically confirmed dengue cases at the Dengue Reference Center in Campos dos Goytacazes, Rio de Janeiro state. The grade of hepatic aggression was established according to the alterations in the aminotransferase levels: grade A - normal levels of aminotransferase; grade B - elevated aminotransferase, with increased levels of at least one of the enzymes; grade C - elevated aminotransferase, with the levels of at least one of the enzymes increased to more than three times the...

Amphotericin B-related nephrotoxicity in low-risk patients BJID
Berdichevski,Roberto Herz; Luis,Luciano Billodre; Crestana,Letícia; Manfro,Roberto Ceratti.
INTRODUCTION: Amphotericin B (AmphoB) is the drug of choice for treatment of severe fungal illnesses; however, it is very nephrotoxic. Modified (less toxic) amphotericins are very expensive. In low-risk patients, saline loading would be enough to prevent significant loss of renal function. MATERIAL AND METHODS: Patients with normal renal function and within the first 24 hours of treatment with AmphoB were prospectively enrolled in the study. Patients in intensive care units or who were using vasoactive drugs were excluded. Saline loads were infused before and after the AmphoB treatment. Blood and urine analyses were made at the beginning and at the end of the treatment. Serum creatinine was repeated 30 days after the end of the AmphoB treatment. RESULTS:...

An adherence trilogy is essential for long-term HAART success BJID
Garcia,Rosa; Schooley,Robert T.; Badaró,Roberto.
Adherence is the milestone of a successful therapy. Over the last decade several authors have addressed the importance of adherence for optimal results of antiretroviral (ARV) therapy. Many health care systems are investing substantial resources to make available contemporary antiretroviral therapy. Despite the large investment in medications, insufficient investments have been made into an integrated adherence component to maximize the impact of these medications. Adherence, unlike drug therapy, cannot be defined as a single method with a defined prescription or formula. Instead, it is the result of a complex interaction between the patient, a prescribed medication and the health system. Many reports are available analyzing each of these components. We...

An epidemiological study of Haemophilus influenzae at a Brazilian day care center BJID
Bonifácio da Silva,M.E.; Marin,J.M..
Day care centers are a relatively new phenomenon in Brazil that bring together large numbers of young children susceptible to contagious diseases. Haemophilus influenzae (Hi) is an important infection in the age range of those attending day care centers. In the present study, the carriage rate of Haemophilus influenzae was identified in 38 day care attendees age 6 to 37 months, and 23 staff members, at a day care center in Ribeirão Preto-São Paulo, in 1997. To identify the carriers, two nasopharyngeal swabs were collected; one in July and one in December. The rate of H. influenzae carriers among the children was 77%. Only 2 of 23 staff members (9%) had Hi. Among the children, there were 58 isolates in the two sampling periods; 6 of the Hi were serotype b,...
